Target Audience
Obstetricians, family practice physicians, emergency physicians, and residents, as well as midwives, obstetric nurse practitioners, perinatal nurses, emergency nurses, and other interested healthcare practitioners who are involved in the intrapartum care of pregnant patients.

Description
The second stage of labor creates a physiologic challenge to both mother and fetus. Oxygen delivery is reduced while oxygen consumption increases during this phase of labor. Certain interventions may help or hinder oxygenation. Using closed-glottis (Valsalva) pushing is now considered acceptable along with open-glottis pushing. However, the practice of prolonged, coached pushing is discouraged due to the impact to PO2 and perineal tissue damage. Current recommendations are to encourage the patient to push as is most comfortable and effective for them. This course identifies factors that increase risk exposure and barriers that may make malpractice allegations more likely. Analysis of a case study shows how mismanagement of the second stage can lead to serious reportable events that place the providers, hospital, and health system at risk for malpractice claims.
Learning Outcomes/Objectives
Upon completion, participants should be able to:
- List 3 factors/barriers that increase risk exposure and make malpractice allegations more likely during the second stage of labor.
- Use interventions that maximize maternal-feal oxygen delivery and minimize oxygen consumption when managing the second stage of labor.
- Design or modify a hospital second stage labor management protocol/policy based on current evidence from high quality studies and professional recommendations.

Error in the mental action or process of learning can lead to adverse events stemming from inaccurate or incomplete thinking, perceiving, experiencing, recognizing, remembering and judging.
